After the IEP team has determined the student is Eligible and will be receiving services, submit a Student Change Form. Once your District/SELPA receives your request, they will change the students status.
Please Note: Only students who will be receiving services should be made Eligible. If the student is found to be Eligible, but will not be receiving services, the student would not be changed to an Eligible status. (See FAQ: An Initial has been held. The student did Qualify for SpEd, but will not be receiving Services. What do I do next?)
1. From the upper navigation bar of the Homepage, click on the Student Change Form link.
2. Select the student’s name from the drop down and click the Go button.
3. From the Select Request drop down, select CHANGE ELIGIBILITY STATUS OF PENDING STUDENT.
4. Respond to the follow up questions
4. From the Status drop down select Eligible: Will be receive services (IEP/ISP/ISP)
5. Enter any additional comments, if needed.
6. Click the Submit button.
You can check the status of the request by clicking on the Eligible/Providing Services link, under Requests Submitted to District/SELPA. Once the request is completed, the Case Manager will be able to Affirm/Attest the IEP.
