Navigate to the STUDENT CHANGE FORM link located in the the Student section of the Upper Navigation Bar on the home page.
From here follow the instructions below:
1. Select the student’s name from the drop down list
2. Select CHANGE ELIGIBILITY STATUS OF PENDING STUDENT.
3. After that is selected, another drop down will appear and here you will want to select ASSESSMENT NOT COMPLETED.
4. Use the optional Comments box to add additional information to your request.
4. Click the Submit button.
At this point, the district will be notified of this request, once processed the student will be removed from the caseload.
