A: Yes. To ensure claims are billed correctly, we will need the following information:

  • The effective date that Dr. Ernest Almendarez begins signing the ORP prescription letters. This lets us know when his NPI can begin being used for OT/PT billing.
  • A copy of the signed ORP contract for our records.
  • The date that will be recorded in each student's file as the effective date for services under Dr. Almendarez's ORP.

Important reminders:

  • Practitioners may only use Dr. Almendarez's NPI for OT/PT services beginning on the date he signs each student's prescription and for services provided from that date forward.
  • Claims cannot be billed retroactively using his NPI for services provided before the prescription was signed.
  • ORP prescriptions must be renewed annually, or sooner if there are changes to the student's prescribed services before the one-year anniversary.
